How does it work if I don't know how much money I will make with my game? Is it fine to start with the free Personal version and then later upgrade to the Plus or Pro licenses when/if I exceed the revenue capacity?

And how would the upgrading process work in practice?

Yes, that's fine. And you'd just purchase the new license - there isn't a new version to download or anything - just buy the license and apply it in Unity.

The revenue limit is all built around the previous financial year. So you don't need to upgrade until after you have actually made the money.
