I think it would be a good idea if unity allowed for developers to include a template photosensitivity/ epilepsy warning screen at the beginning of a game, the same way the unity splash screen can be switched on or off.

Why not just have developers do it themselves if needed?

I guess a lot of developers won't be aware of it. Not many indie games come with such warnings

Takes less than 5 min to add one scene that shows such image or text for few seconds and then loads the main scene. No need to have this as maintainable engine feature.

I agree with this and it's not something that Unity would be likely to include for reasons of legal liability. Not to mention that it would only impact some developers. This is the type of screen that's better off left to the developers and not to Unity itself.

Custom splash screen is happening anyway, you could shoehorn that there

Unity already allows that. You can implement your own intro/logo screen and put the warning there.

You can. Just include an empty screen with a simple UI set up with the warning.

Its also worth noting that laws are varied world wide on the required warnings. No point to put a warning in place unless you are legally required to do so.
