Bug Multi-Aim Constraint flat out not working

I'm not sure what is going on, but out of nowhere, some of my Multi-Aim Constraint(s) have stopped working. On some objects it works, and on others it doesn't. However, my IK rig structure is pretty much the same as all other.

The assets I'm using Meshmint's second monster pack and this persists even if I was to delete all components and re-rigged it. For example, on the wolf character, the Multi-Aim Constraint(s) stopped working, but on the dragon avatar, it works fine.
All the head bones have the same orientation; -X up, Y forward.
And they're set up all the same on the head bone (for each model):
    Um.. Help? Any ideas?
There's likely something internal going very wrong here...
