Help with a name

Ok this is my first game I have been trying to make and I want to make it so the user is able to make a name and I am trying to make it so when he puts that name it appears on top of the car.

I heard that you have to create a form and that form adds information to the game. Where can I find a tutorial on how to make a form or is there another way of doing this?
