Fog is not required.
Humm, weird. Ive been testing a prefab and the material is correctly created. The other problem about lightmapped levels... I don't see the relation between lightmaps and foggy lights. Works perfect for me.
Anyhow, I will upload what I currently have, maybe I made changes months ago and never uploaded it.
Be sure to use the latest version (1.6)

Make sure that your camera is generating the depth texture. I think I included a script called "EnableDepthInForwardCamera", or something like that
Have you tried my demo scene first?
The only param you have to tweak for this is the offset.
