Cannot create unity ads account

I am attempting to create a unity ads account. I already have a UDN account. I click on the "UDN or signup" option, which leads me to a screen where I select "Earn Money With Your Games."

I can get no further than this. The browser white-screens and gives a message:

{"message":"User validation failed"}

I have tried this with 3 different browsers (Chrome, Firefox, Safari) all with the same result. At a loss here at what to do.
