Which unity version is used? Do you use vulkan or OpenGLES? (if vulkan, do you use graphics jobs) Can you share URP asset settings and renderer...
When does it happen? Is it from your code or from the engine? Does it give any issues?
Any errors or logs? Do you have multiple cameras on the scene? Which unity version and plugins/packages are used? How is your scene/VR player set up?

Do you actually get an issue when using the older OpenXR version without the quest 3 check? Maybe with a custom manifest?
Start with the basics, like the VR pathway on Unity learn. You can probably use the ray interactor combined with the on hover event with XRIT
